Pre-conference for Home Care Clinicians: Integrated Chronic Care Management

Train the Trainer Education & Certification for Home Care Nurses & Therapists: In response to numerous requests, the creators of the award-winning Home-Based Chronic Care Model ™ have developed a concentrated one-day course to prepare homecare clinicians for the future of integrated chronic care delivery.

Participants will receive one day of classroom instruction and interaction, supplemented by five hours of web-based training and online testing. They will be awarded a certificate designation as an Integrated Chronic Care Specialist Trainer and receive 12 contact hours for nursing.

Participants will also be able to immediately implement these concepts in their agency and begin the process of demonstrating value as expert Home Care providers as well as the RIGHT provider of patient centered high quality chronic care across the healthcare continuum.

This enhanced clinician skill set, and the resultant care delivery model, positions agencies to be a provider in a variety of nontraditional home care roles, such as: a health coach in transitions of care, a prepared proactive practice team in Patient Centered Medical Home, a care coordinator for long term chronic care management, and a valuable partner for Accountable Care Organizations.
